CDC Health Equity & Community Rebuilding Initiative

What is Health Equity?

Health equity means that everyone has a fair and just opportunity to be as healthy as possible.

This requires removing obstacles to health such as poverty, discrimination, and their consequences, including powerlessness and lack of access to good jobs with fair pay, quality education and housing, safe environments, and health care.

The Program

With CDC/ Kellogg funding, participating county and tribal health councils work on community collaboration and engagement to identify and address local vaccine equity goals in Year One and focus on community health building and equity in Year Two.  

Key partners include:

  • New Mexico Alliance of Health Councils

  • New Mexico Dept. of Health

  • Center for Health Innovation

  • Community Health Work Initiative

The grant is for two years, with the possibility of 3rd-year funding for sustainability.

The work is centered around COVID-19 outreach, education, and support for vaccination activities in rural, tribal, and higher-risk populations. NMAHC staff will be coordinating support for health councils, assessing COVID-19 activities, successes, challenges, and contributing to a comprehensive report detailing how the project can be replicated and sustained in NM counties and tribal communities.

Training and Technical Assistance Schedule.

Community Health Councils participating in the Health Equity & Community Rebuilding Program are expected to attend a number of training and technical assistance sessions. These sessions are also open to other health council members and interested community members.

Trainings happen on the 1st and 3rd Monday of every month from 1-3PM. Find updated information about upcoming trainings on our Events page.

Community Engagement Resources.

NEW! Visit the New Mexico Department of Health Community Resource Center. Go to https://getthefacts.vaccinenm.org/chw/ for the one-stop, up-to-date NMDOH site for COVID-related registration, flyers, training materials, flip charts, data, and trusted resources in both English and Spanish for community health workers and other practitioners working in the field on vaccine equity. The latest: COVID-19 Explained for Kids. COVID-19 vaccines and the vaccination process explained for kids, answering such important questions of "What does the vaccine do?" "What does getting a vaccination feel like? In English and Spanish.


US Surgeon General’s Community Toolkit for Addressing Health Misinformation.

Misinformation: information that is false, inaccurate, or misleading according to the best available evidence at the time.

Misinformation has caused confusion and led people to decline COVID-19 vaccines, reject public health measures such as masking and physical distancing, and use unproven treatments.

The Trusted Voices effort asks community members throughout NM to share pandemic response messages via social media. Listen to the voices of people you trust from the Trusted Voices playlist. Communication Materials and Resources for Spanish-speakers.

Protecting Immigrant Families Coalition posted a Spanish language video providing facts about COVID-19 vaccinations that addresses questions about immigration status (documentation is NOT required for vaccinations). Watch the video.


El Sol has a new “Time to Heal” COVID-19 Song and Tool Kit. This project will guide community health workers and promotores to help promote healing in their communities during this pandemic. Find resources such as a weekly mental health check-in plan, a guide for self-care planning, healing plans, an emotional healing map, support group outlines, real stories, song lyrics, and fact sheets related to mental health.


Santa Fe County Health Services Division created a compilation of all their public awareness materials developed related to Covid-19, including mask,  distancing, and vaccination.  There are both English and Spanish materials.  Also, check out their video channel.


The Center for Disease Control and Prevention have posted a wide range of communication materials, including videos, print resources, PSAs and more in multiple languages.
